Introduction

This is a case analysis for EB, a 68-year-old African female who is reports of worsening of her vision over the past few days. She reports having glaucoma and hypertension. She is on hydralazine 50mg, Metoprolol 200mg, Lisinopril 40mg, and HCTZ 25mg. It seems she has not been complying with her medication. Her blood pressure is 182/99mmHg and her pulse rate of 84 beats per minute.

How to Build a History

As the certified nurse on duty, I took the patient’s history upon arrival at the hospital. I greeted and welcomed the patient into the consultation room and ensured she was comfortable observing the confidentiality of the conversation. I asked her about the language she was comfortable to communicate with and she proceeds to explain her complaints. During the interview, I was actively listening, maintaining eye contact with the patient, and observing the non-verbal cues to understand what she meant.
